Supports & Services

Tri-Generations will meet all SCL requirements for the services and supports provided to the individuals in our program.  The following is a list and description of services/supports provided:

Case Management
This service helps individuals and their families to develop a plan to meet the individual's needs. The case manager works closely with the individual to monitor services, advocate on their behalf and ensure the individual is satisfied with services.
 
Residential Supports
Individuals receiving services may choose to live in a variety of places, including a foster family home or a staff residence (a small three-person individualized home). Residential supports provide twenty-four hour supervision and training in activities such as laundry, routine household care, self care, shopping, money management, socialization, and leisure activities.  Also, folks receiving residential services may choose to live in their own homes.
 
Adult Day Training (ADT)
This service provides training during the day for individuals to learn how to use money, learn job-related skills, use community resources, learn how to communicate better and how to interact appropriately with others.  This can be provided at a facility (on-site) or in the community (off-site.)
 
Supported Employment
A job developer assists the individual to find a job in the competitive market that is best suited to his/her talents. When the individual finds a job, a job coach will work with the individual at his/her work place to assist the person in learning the skills needed to do the job.
 
Community Living Supports
Individuals, who live in their own homes or with a relative, receive support, training or assistance in such activities as shopping, household care, and participating in community activities.
 
Behavior Supports
These supports assist the individual to change maladaptive behaviors and learn more appropriate and socially adaptive behaviors. This in turn helps the individual to become more a part of their community and achieve their dreams and desires for their life.
 
Psychological Services
This service includes psychological testing for diagnosis, evaluation and treatment. Individuals with a dual diagnosis, such as a mental health diagnosis along with their developmental diagnosis may need this service to coordinate treatment for mental health issues.
 
Occupational Therapy
This therapy service may include adapting tasks or the individual's environment to help the person become more independent.
 
Physical Therapy
This therapy service helps prevent, correct or minimize physical handicaps. It also helps to reduce pain and improve the body's ability to function.
 
Respite
This service provides a time of rest or relief for the people who normally care for an individual. Respite services are only available to individuals living with relatives or in foster family homes.
 
Speech Therapy
This therapy service helps individuals to improve their communication and oral motor skills.
